Becoming a New Provider

Change of Ownership (CHOW) Licensing

If you have no experience with AFH’s, and you’re starting one from scratch, this is the package for you. This package has four phases. The first is to get your AFH ready to be licensed, the next to get operations ready, the third to walk you through the inspection process, and the last to get your home running.

This is the package for starting fresh with a currently licensed AFH that you are taking over. We take you through three phases: how the AFH can be made profitable, how to get the AFH operational, and how to keep your records in order.

PHASE 1:
To Start Off

We begin with a thorough
pre-license home/property inspection. If you don’t have
a house/home in mind, we will/can help you find one with potential.

Then we consult with you on the Washington Administrative Code (WAC) and training requirements. We will assist in opening your business, getting your city business license, and your federal employer identification number (FEIN).

We help you prepare to be a small business owner and to open the AFH.

We assist in contacting your local building official to determine/access the process and cost to complete their Building Inspection Preparation Checklist.

We will be with you when you meet with the contractor who is remodeling the home to ensure changes are up to building code specifications.

PHASE 2:
The Operational Side

First is the set-up of medication, resident admission, and hiring staff.

We’ll lead you through the drafting of a policy and procedure manual.

We start you on how to properly organize business and care records.

It’s time for setting up the property for an AFH license.

We’ll help you submit the AFH Licensing Application.

We work with you to prepare for the AFH Initial Licensing Inspection.

PHASE 3:
The Licensing Inspection

Now we work with you and the state licensor in order to submit all the required policies are submitted and that you pass licensing.

We will have two of our staff available on the day of the inspection to be on-site with you.

PHASE 4:
Opening in a Successful Way

To help you start up we will review admission for the first four resident records.

We will also look over staff records; training, background checks, orientation, etc for your first two staff numbers.

Finally, we’ll get you started Medication Reconciliation (the process of creating accurate lists of medications each residence takes).

We’ll help you with hiring staff, admitting residents, and handling medication.

We can teach you how to create a policy and procedure manual.

Time to implement what was started in Phase 2.

Also a full inspection of the home.

We’ll review the staff and resident records.

The Complimentary Package

This package is an addition to the other packages but is also offered separately. Residents are the most important variable in the success of your AFH. Sometimes a resident won’t be a good fit for your home, and sometimes it can be difficult to find ones that will be. We can be of assistance!

We’ll help place clients in your AFH.

We will assist in the discharge of a resident if the discharge meets WAC guidelines. Then we will send a referral to one of the other AFH’s that are contracted with us.

This package is an addition to the other packages but is also offered separately. Residents are the most important variable in the success of your AFH. Sometimes a resident won’t be a good fit for your home, and sometimes it can be difficult to find ones that will be. We can be of assistance!
